As mentioned on the history page: The 59th Regiment poor Herbie lost his life in a silly training exercise.
I asked about BSM Herbie Wells on a forum and very quickly uncovered some information.
I still find it amazing that the good folk on the ww2talk forum are able to come up with so much information about unremarkable servicemen & women from the WW2 era.
In an attempt to find out who he was and what kind of accident took his life, I posted my request for information here: http://ww2talk.com/index.php?threads/information-request-6000469-bsm-herbert-charles-herbie-wells-59-haa-regiment-ra-18-07-1940.90293/
To have started the war in 1939 with the rank of BSM, Herbie must have been an experienced career soldier, but too young to have served in WW1.
Looking through an aiming disc while a trainee points and fires an unloaded rifle at your eye, isn't my idea of fun. But like many accidents, having repeated this exercise thousands of times, it only takes a silly mistake to turn from routine to tragedy.
Anyway, I have now updated The 59th Regiment history page on this site to include some of this new information.
